The Ho-Chunk Health Care Center is
planning an Adolescent Health Fair for July 12, 2004. This would be a
great day on which to schedule that sports physical and receive additional
information on topics specifically pertaining to teens.
Playing sports is a great way to have fun and stay fit. For adolescents
who participate in an organized sport, a complete physical is required
every two years. It is important to schedule all physicals now, as state
law requires that all physicals and paperwork be completed before a
student is allowed to practice. If the physical is current (within 365
days), only an update sheet need be signed by the parent and submitted
along with the permission to participate form.
Clinic providers may see adolescents periodically throughout the year for
acute illnesses; however, these clinic visits are not considered complete
physicals and cannot be used to complete sports physical forms. If your
child wishes to participate in school sports and needs a physical, please
call 284-9851 to schedule an appointment now.
Also, public school systems require physicals to ensure that children are
well and immunizations are up-to-date before children enter kindergarten.
Immunizations are provided, as needed, at the time of each physical.
Please inform your child's medical provider of food and medication
allergies, diabetes, asthma or any health concern that may impact your
child's school day. Medication orders and/or health care plans need to be
developed before children enter school.
Parents are advised to schedule physicals and immunizations early to avoid
the last-minute rush. |
