Sports, Camp, & Kindergarten Physicals

Sports & Camp Physicals
Playing sports is a great way to have fun and stay fit.  If you play team sports, a physical is required to ensure that you are in good enough condition to play the sport you want to participate in.  Your body works hard when you run fast, kick, jump, and throw.  That is why a sports physical is important.  Football, soccer, volleyball, and basketball players, and cross-country runners, should plan ahead for sports physicals.  There are two parts to a sports physical: medical history and physical exam.  The medical history includes questions about illnesses and injuries you have had.  The provider will also want to know about medical problems that run in your family and medicines you take.  During the physical exam height and weight are documented and a medical provider checks lungs, heart, blood pressure, eyes, and ears.  Deadlines are crucial, as state law requires that all physicals be completed before a student is allowed to practice.  A new permission form is required for every season.  If the physical is current (within 365 days) an update sheet, signed by the parent is submitted along with the permission form.  Camps may also require physical exams annually or every two years.  Even though we may see your child periodically during the year for other reasons, theses visits are not considered complete physicals and cannot be used to complete your child's form.  If your child desires to participate in school sports or attend camp, and if a physical exam is needed, please call early to schedule an appointment.

Kindergarten Physicals
School systems require kindergarten physicals to ensure that children are well and immunizations are up-to-date.  Exams include physical and developmental assessments, hearing and vision checks, hemoglobin, blood sugar and UA and lead testing for children under the age of 6.  Immunizations will be provided if needed at time of the physical.  Please inform your child's provider of food allergies, diabetes, asthma or any health concern that may impact on your child's safety.  Mediation orders and/or health care plans need to be developed before medically fragile students may enter school.  Parents are advised to schedule physicals and immunizations before school starts to avoid the last-minute rush.
